If you are a new patient, once your appointment is confirmed, you will receive an email with a link to complete the new patient paperwork. If you would like to use insurance, be sure to include that information in your forms so I can verify your benefits prior to your visit. If you have any questions, or the online system does not work for you, please call 503-575-1275.
New Patients: All visits must be scheduled 48 hours in advance, and paperwork must be submitted 24 hours in advance to give me time to review your answers, verify your insurance, and reach out to you for clarification if needed. If you have questions you would like answered prior to scheduling your appointment, please schedule a free 20-minute consult.
Home Visits: Please call our office to schedule home visits. Do not use the online scheduling system. Please Note- We are not able to bill insurance for home visits and they are not eligible for the 25% prepay discount.
