If you are a new patient, once your appointment is scheduled, you will receive an email with a link to complete your new patient paperwork. If you would like to use insurance, please be sure to include that information in your forms. This way, I can verify your active coverage and my network status prior to your visit. If you have any questions, or the online scheduling system does not work for you, please call 503-575-1275.
You can schedule your visit 48 hours in advance. I ask that all paperwork be completed and submitted 24 hours before your appointment. This gives me the time to review your answers, check your insurance, and reach out to you for clarification if needed.
Before you enter the office, I personally review all your paperwork, create your chart, and have invested 30 – 60 minutes to your care. I do this so I can be ready for you and our visit time is devoted to your care instead of paperwork, research, and long hold times with your insurance company.
